WHERE IS THE ONSITE CARE CLINIC LOCATED?

The clinics are located at:
2755 NC-66, Kernersville, NC 27284 (employees and eligible family members)
201 N Pendleton St, High Point, NC 27260 (employees only)

WHO IS ELIGIBLE TO USE THE ONSITE CARE CLINIC?

The clinic is open to employees, dependent children ages 2-25, spouses, or certified domestic partners. You must have your photo ID to be seen in the clinic. Eligible family members may only use the Kernersville location.

WHAT ARE THE HOURS OF OPERATION FOR THE ONSITE CARE CLINIC?

8:00 am – 4:30 pm - Monday-Friday. The clinic will be closed for lunch from Noon - 1 pm. The last appointment will be at 4 pm.

HOW DO I MAKE AN APPOINTMENT TO BE SEEN AT THE CLINIC?

You can call 336-893-0013 or online to make an appointment online. 

DOES THE ONSITE CARE CLINIC TAKE WALK-INS?

Though we will have a limited number of same-day walk-in appointments, we HIGHLY recommend that you make an appointment to avoid waiting and to ensure that you are able to be seen.

WHAT HAPPENS IF I’M LATE FOR MY APPOINTMENT?

Patients arriving later than 15 minutes for their scheduled appointment may be asked to reschedule.

AM I ALLOWED TO USE THE ONSITE CARE CLINIC DURING MY WORK SHIFT?

Yes.  If you come to work and are feeling ill, you may make an appointment at the clinic by phone at 336-893-0013 or online. Seek your supervisor’s permission before leaving work. 

IF I USE THE ONSITE CARE CLINIC DURING MY SHIFT, AM I REQUIRED TO USE SICK TIME?

It depends. If your shift has not ended once your appointment has ended, you are expected to return to work. If you do not return to work, you will be required to use your sick leave for the duration of your shift. If you do not anticipate returning to work, please follow your department’s normal call-in procedures.

AM I ALLOWED TO COME TO THE ONSITE CARE CLINIC ON MY DAY OFF OR WHEN I HAVE CALLED IN SICK?

YES! Please make an appointment as you would on a day that you were at work.

DOES THE ONSITE CARE CLINIC OFFER VIDEO OR TELEPHONE VISITS?

YES! You may schedule by calling the clinic at 336-893-0013.

WHY AM I SEEING A BENEFIT CREDIT AND DEDUCTION ON MY PAYCHECK?

DO I NEED INSURANCE TO BE SEEN AT THE ONSITE CARE CLINIC?

No, the clinic is open to all employees, dependent children ages 2-25, spouses, or certified domestic partner with or without insurance for the majority of services. Some labs or other off-site services may be billed to the Ralph Lauren insurance plan. For those not on the Ralph Lauren insurance plan, some services may be limited. Please call the clinic at 336-893-0013 before coming if you would like more information.

ARE THERE ANY HOLIDAYS THE ONSITE CARE CLINIC WILL NOT BE OPEN?

The clinic will be closed on New Year’s Day, Martin Luther King Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Day, and any additional days Ralph Lauren is closed.

Per IRS regulations, Consumer Plan members who visit the clinic and have not yet met their annual deductible, will see imputed income of $50 on their pay statement. That means you pay taxes on this amount.

WHO HAS ACCESS TO MY MEDICAL RECORDS LOCATED AT THE ONSITE CARE CLINIC?

Only Wake Forest Baptist Health (WFBH) clinicians providing your care will have access to your medical records. They utilize an electronic medical records system, so information can also be shared with your primary care physician. Ralph Lauren or its employees do not receive employee information as WFBH is bound by strict privacy and confidentiality laws governed by the Health Insurance Portability and Accountability Act (HIPAA).

WHO ARE THE PROVIDERS AT THE ONSITE CARE CLINIC?

Our clinic is staffed by licensed providers from Wake Forest Baptist Health.

WHAT ARE THE CREDENTIALS OF THE PROVIDERS?

The clinic will be staffed with a Nurse Practitioner or Physician Assistant, Registered Nurse, and licensed Behavioral Health Counselor.

WHAT ARE THE CREDENTIALS OF THE BEHAVIORAL HEALTH PROVIDER?

Britney Monroe is a Licensed Clinical Mental Health Counselor and National Certified Counselor. She received her Master of Education in Counseling from James Madison University and is a Licensed School Counselor. Britney also has teaching and supervisory experience with masters-level counseling students. Britney specializes in working with anxiety, depression, trauma, grief/loss, stress, self-esteem, relationship and family conflict, emotional regulation, and careers. She aims to provide a safe and supportive space to collaborate and work to help others reach their goals and find inner strength to cope with life challenges.

WHAT SERVICES WILL THE BEHAVIORAL HEALTH PROVIDER?

• Referrals to other behavioral
health professionals

• Anxiety and stress

• Transition and adjustment
• Relationship issues
• Goal setting
• Substance use
• Trauma

MAY I HAVE MY PRESCRIPTIONS FILLED AT THE ONSITE CARE CLINIC?

At this time Wake Forest Baptist Health is not offering onsite pharmacy services. Providers at the clinic will send any needed prescriptions to the pharmacy of your choice.

CAN THE PROVIDER AT THE ONSITE CARE CLINIC WRITE ME OUT OF WORK FOR FMLA?

No. The provider at the clinic can only write employees out of work for three days for illness. More serious concerns that may result in longer time away from work will be referred to a primary care physician of the employee’s choice.

I HAVE A PRIMARY CARE PROVIDER.  CAN I STILL USE THE ONSITE CARE CLINIC?

YES! The clinic is not meant to take the place of your Primary Care Physician. If you need a primary care or specialist referral to Wake Forest Baptist Health or any other local provider, a clinic staff member can assist you.

MY PRIMARY CARE DOCTOR IS NOT WITH WAKE FOREST BAPTIST HEALTH.  WILL HE/SHE BE ABLE TO SEE THE RESULTS OF MY LAB WORK OR ANY OTHER VISIT INFORMATION FROM THE ONSITE CARE CLINIC?

YES. Wake Forest Baptist Health, Novant and Cone Health all use the EPIC Electronic Medical record allowing providers to provide the best care for patients.
